What are the primary requirements of the CQI+ program?

Pharmacy teams must implement an online incident management platform for practice incidents and close calls, complete a validated Safety Self-Assessment (SSA), develop site-specific policies, and conduct regular CQI meetings (at least quarterly).

The data processing fee is a separate charge by ISMP Canada and is not included as part of Pharmapod’s services or fees.

How often must the SSA be completed? The self-assessment must be documented by the licensee at least every two years, or within six months of a licensee change.
